Before You Deploy Agentic AI, Fix These Two Things First

The promise of agentic AI in safety is real. Are there fields filled with placeholder text—"TBD," "unknown," "see notes"—that suggest the person entering data didn't have the information they needed? That's a system design problem, not a data problem.

The Root Cause

The root cause is almost always human behavior and system design at the moment of capture. When a field worker fills out a form on a tablet after a 12-hour shift, they're not thinking about downstream analytics. They're thinking about getting home.

If the system doesn't force them to fill in a severity field...

They won't.

If the default is "low risk"...

They'll leave it.

If the form has 47 fields and only 3 are actually required...

The other 44 become noise.

Fixing Data Quality

Fixing this means going back to that moment:

Redesign forms so that critical fields are genuinely required, not just recommended.
Set smart defaults that reflect reality, not wishful thinking.
Train field teams on why the data matters—not just that it does.
Accept that this is ongoing work, not a one-time fix.

The organizations that have done this well don't talk about it much because it's not glamorous. They just have better data.

Diagnosing and Solving Your Data Access Problem

Start with an audit. Map every system where safety-critical data lives. Your EHSMS. Your CMMS. Your asset management system. Your incident tracking tool. Your training platform. Your inspection software.

For each system, ask:

Can this system export data in a structured format?
Does it have an API?
Can external systems read from it in real time, or only through manual exports?
Can it connect to other systems, or is it an island?
